Dietitian Jobs at UVM Health can include: Clinical Dietitian - Outpatient - Team members in these dietitian jobs direct, and provide medical nutrition therapy (MNT) to outpatients in designated areas/programs. Responsible for nutrition screenings, assessments, the prescription of nutrition orders, ongoing monitoring of patients’ care, and documentation of care plan in accordance with department and hospital policies. Community Health Team Registered Dietitian - Team members in these dietitian jobs will be working directly with the Primary Care Providers (PCP) in the Patient Centered Medical Homes (PCMH) within this Hospital Service Area (HSA) to extend the care offered by the PCP to their patients with risk profiles or health problems related to diet, nutrition, obesity, and weight maintenance. Registered Dietitian - Nutrition Services - Team members in these dietitian jobs provide nutritional care and information to patients in and outside of the organization including conducting screening and assessments of patients as ordered by the physicians, providing dietary counseling, preparing meal plans and nourishment programs for special needs of patients, and conducting regular meal rounds.
