The Assistant Clinical Manager (ACM) is a Registered Nurse leadership role that supports Home Health & Hospice Clinical Managers and Directors in delivering safe, compliant, high-quality home health and hospice care across community-based services.

The ACM provides clinical oversight during assigned coverage (including weekends/holidays and off-hours as needed), supervises and schedules designated staff, coordinates care delivery and handoffs, and serves as a point of escalation for clinical and operational concerns.

The role partners closely with interdisciplinary and administrative teams to ensure continuity of care seven days a week and adherence to organizational policies and applicable regulations.

EDUCATION
Associate or Bachelor’s degree in Nursing from an accredited program.
Current, unrestricted RN license in the state of practice and BLS certification.
Hospice and Palliative Nursing certification (preferred) or willingness to obtain within 1 year

EXPERIENCE
Three years’ experience as a Registered Nurse in Home Health and/or Hospice/Palliative care; 
Two years progressive leadership/supervisory experience. Consideration may be given for a combination of education and experience from which comparable knowledge and skills are acquired
Demonstrate and apply working knowledge of hospice/home health regulations, standards of practice, and clinical workflows to promote compliance, efficiency, and high-quality outcomes; Apply clinical judgement to balance patient needs, regulatory requirements and organizational policies in care planning and delivery.
Maintain productivity and quality of work while managing frequent interruptions and multiple concurrent responsibilities
Demonstrate effective written and verbal communication skills when interacting with staff, leadership, patients, and interdisciplinary partners.
