What You’ll Do

Manage a panel of approximately 6–8 patients per day
Assess and manage acute changes, chronic conditions, and post-hospital transitions
Lead daily rounding, including review and triage of overnight events with nursing staff
Support safe and efficient discharges back to primary care providers
Collaborate closely with physicians, nurses, rehab therapists, and care managers
Schedule
0.6 FTE (30 hours/week)
Three 10-hour shifts per week (typically 7:00am – 6:00pm)
Rotating weekend phone call (7:00 AM – 7:00 PM, from home); paid call coverage
What You Can Expect Here
A collaborative, interdisciplinary care model across a 150+ bed facility
Care for both long-term residents (90–100) and short-term rehab patients (50)
A highly team-oriented environment with strong nursing and leadership support
Meaningful continuity of care
Qualifications
NP or PA with (or eligible for) Vermont licensure
BLS certification required
Preferred:
2+ years of experience in primary care, hospital medicine, urgent care, or post-acute care
Comfort managing medically complex adult or geriatric populations
