JOB DESCRIPTION:

The Primary Care Practice Supervisor is responsible for providing the daily leadership necessary to achieve the Ideal Primary Care Patient Encounter, ensure continued achievement of NCQA accreditation as a patient centered medical home, and create an environment where primary care staff and providers are highly engaged in the ongoing process of achieving excellence in the patient care experience, health outcomes, cost, and teaching the next generation of health care professionals. The Primary Care Practice Supervisor, in partnership with the primary care site’s MD Site Leader, have accountability for the performance of individual care team members as well as ensuring the goals for the site and primary care services overall are achieved. The Primary Care Practice Supervisor will be an active member of the Medical Home Leadership Team (MHLT) focused on continually improving and standardizing the key processes core to delivering highly reliable service and care. The Primary Care Practice Supervisor carries out responsibilities at the primary care site(s) through the management of operations which include, but are not limited to: patient access, scheduling, insurance precertification, referral management, resource utilization, documentation, charge capture, service quality, compliance with accreditation agencies (e.g., The Joint Commission and NCQA), clinical standards of care as defined by MHLT and clinical site staffing according to our primary care staffing model. In addition to these responsibilities, the Primary Care Practice Supervisor must have an intense commitment to high level customer service, and the ability to foster that culture within the site or sites they manage. The Primary Care Practice Supervisor will provide administration direction and coordination of current and long range policies, procedures and programs throughout the Medical Group and the entire Organization.

EDUCATION:

Bachelor’s degree in business administration or health care or related field, with 3 to 5 years of relevant leadership experience preferred or Associate’s degree or Diploma in Nursing with 5 years of relevant leadership experience required.

EXPERIENCE:

Knowledge of physician practice operations and workforce development. Working knowledge of health care financial environment, professional revenue cycle, electronic medical records, practice support systems and related business operations. Knowledge of patient centered medical homes strongly desirable.
